Required Upon Application:
- Teacher Recommendation Forms given to current/previous school:
The teacher filling out the recommendation form(s) may send by mail to 5800 Westheimer Rd, Houston, TX 77057, or email directly to Memorial Lutheran School.
- Copies of most recent report card and any testing scores
- Transcript Request - to current/previous school
- Copy of immunization records - requirements K to 8; requirements Infants to PreK
After acceptance, the following forms and documents are required before a student attends (please download):
sports physical form (5th-8th grades submitted by the first day of school)
Copy of birth certificate
