Required Upon Application:
- Teacher Recommendation Forms from current/previous school:
The teacher filling out the recommendation form(s) may send by mail to:

or email directly to Memorial Lutheran School.

Copies of most recent report card and any testing scores
- Transcript Request — from current/previous school
- Copy of immunization records — requirements K to 12; requirements Infants to PreK
Required After Acceptance:
- healthcheck list
- allergy form
- signed healthcare professional's statement
- sports physical form (5th-8th grades submitted by the first day of school)
- Copy of birth certificate
